What is the difference between a Nurse Care Manager and Caregiver?

A Nurse Care Managers is trained to manage a range of health, functional, emotional, psychological, legal and financial issues. This includes developing a care plan, cordinating resources, overseeing caregivers, managing medications, and monitoring client needs. Caregivers provide hands-on assistance with dressing, bathing, grooming and toileting. Caregivers will also prepare meals, do light housekeeping and provide companionship.


Will you receive reports?

Yes, we will provide a status report after each visit, medical appointment, or significant event.


Are we available for emergencies?

Yes, we are on call 24/7 for emergencies.


What is a Care Management Assessment?

Our Nurse Care Manager wil visit your loved one to perform a holistic Care Management Assesment in the comfort of their home, facility or hospital room. The Nurse Care Manager will evaluate the client’s:

  • Physical, mental, emotional and spiritual well-being
  • Safety
  • Unmet needs
  • Cognitive status
  • Medication
  • Medical providers

Based on needs identified during the assessment, a Care Plan is develops.


How much do our services cost, and who pays?

Services are billed on an hourly basis to our clients. Fee schedules are available on request. Our services are private pay and are not covered by medical insurance. Many long-term care insurance policies cover care management (care coordination) services; refer to your policy to see if care management (care coordination) is a covered benefit. The individual Care Plan will be developed and reviewed with the financially responsible parties prior to implementation. Our services are billed monthly; which includes a detailed invoice outlining the service provided.


Is there any long term commitment required?

No, our services are tailored to meet the needs of the client. The degree of involvement can be increased or decreased as needs change and can be terminated at any time.

Do Care Managers provide clinical care?

No, we do not provide hands-on clinical care. Our experience and specialized knowledge allow us to create personalized, holistic Care Plans based on the client’s individual situation and needs and will arrange for the appropriate provider to adress such needs.
