As part of celebrating the month of May as Aging Life Care Month, we would like to help educate our visitors on what it is that Aging Life Care Professionals, also known as Geriatric Care Managers, do to help aging persons and their involved family members to plan for care and solve aging challenges.

An Aging Life Care Manager is someone who has a background in healthcare and can assist the family or responsible person to help the client. Whether acting as an advocate, developing a plan of care to guide care, or managing care ongoing, an Aging Life Care Manager can be of assistance. Because Care Management is a private service, it can be tailored to each client’s situation and needs. Most Care Managers service the older adult, 65 and older, but some Care Managers service clients 18 and above.

Desert Care Management is a nursing model with a Nurse Care Manager and a Certified Nursing Assistant as a Care Manager Assistant. Because we are a nursing model, we can manage medications and communicate with healthcare providers at their level. We also work with clients 18 and above.

Our Care Management services include:

Assessment and Care Plan
An individual Care Plan will be developed by identifying the needs of your loved one. This Care Plan is a road map for the resources and services that are required. Recommendations of medical, legal, and financial providers will be made as appropriate.

Ongoing Care
Our Nurse Care Manager will manage the needs of your loved one, adjusting the Care Plan as needed. This can include accompanying and advocating for your loved one during medical appointments, as well as updating the family regarding their health.

Medication Management
Our Nurse Care Manager can set up and monitor medication for your loved one. This includes managing refills and communicating with doctors as needed.

Crisis Intervention
Crisis Intervention includes an Assessment and Care Plan but is expedited in a much more rapid time frame due to the nature of the situation. Our Nurse Care Manager is available 24/7 for crisis situations.
